Despite medicine's achievements, medical errors and the difficulty of reproducing research prove medicine is far from perfect. This book provides a critical and historical analysis of medical reasoning that recognizes the constant need for certainty despite the enormous variety of disease, illness, symptoms, and behavior in patients. According to Erwin Montgomery, Jr., medicine depends on logic, balancing utility with certainty, and anticipating errors in judgment.

Ethics of Everyday Medicine: Explorations of Justice examines and analyses the relatively unexplored domain of ethics involved in the everyday practice of medicine. From the author's clinical experience, virtually every decision made in the day-to-day practice of medicine is fundamentally an ethical question, as virtually every decision hinge on some value judgment that goes beyond the medical facts of the matter. The first part of the book is devoted to medical decision cases in several areas of medicine. These cases highlight (...) elements of the current healthcare ecosystem, involving players other than the physician and patient. Insurers (private, commercial, and governmental), administrators, and regulators' perspectives are surfaced in point of care case analysis. Part two contributes to the development of actionable tools to develop better ethical systems for the everyday practice of medicine by providing a critical analysis of Reflective Equilibrium and ethical induction from the perspective of logic and statistics. The chapter on Justice discusses the neurophysiological representations of just and unjust behaviours. The chapter on Ethical Theories follows, describing the epistemic conundrum, principlism, reproducibility, abstraction, chaos and complexity. The following chapter approaches ethical decisions from the logic and statistic perspectives. The following chapter, The Patient as Parenthetical, the author discusses patient-centric ethics, and the rise of business- and government-cetric ethics. Obtaining informed consent is a cornerstone of biomedical research, yet participants comprehension of presented information is often low. The most effective interventions to improve understanding rates have not been identified.

Combinatory logic started as a programme in the foundation of mathematics and in an historical context at a time when such endeavours attracted the most gifted among the mathematicians. This small volume arose under quite differ ent circumstances, namely within the context of reworking the mathematical foundations of computer science. I have been very lucky in finding gifted students who agreed to work with me and chose, for their Ph. Suicide among Veterans experiencing or at risk for homelessness remains a significant public health concern. Conducting research to understand and meet the needs of this at-risk population remains challenging due to myriad factors. To address this challenge, the United States Department of Veterans Affairs convened the Health Services Research and Development Suicide Prevention in Veterans Experiencing Homelessness: Research and Practice Development meeting, bringing together subject-matter experts in the fields of homelessness and suicide prevention, both from within and outside of VA. (...) During the meeting, attendees identified 10 potential research priorities at the intersection of suicide prevention and homelessness. After the meeting, Delphi methodology was used to achieve consensus on the relative importance of the identified research domains. Through this iterative Delphi process, agreement was reached regarding the need to increase understanding of barriers and facilitators to suicide risk assessment and emergency intervention for Veterans experiencing homelessness by examining the perspectives of both Veterans and healthcare providers. Elucidating the complex relationships between risk periods, subgroups, suicide means, and drivers of suicide among Veterans experiencing homelessness was also considered a top priority. This article documents the Delphi process and provides a research agenda for researchers, funding agencies, and policymakers to prioritize the most relevant and potentially impactful research domains aimed at preventing suicide among Veterans experiencing or at risk for homelessness. The distinctions between A-series and B-series, between synchronic and diachronic identity and between perdurance and endurance are basic in the philosophy of time; yet they are flawed. McTaggart’s claim that the B-series is static and that a series has to be changing to be really temporal arises from a misunderstanding of temporal relations and of the task of ontological analysis. The dynamic appearance of the A-series results from the incompleteness of the analysis. “Synchronic identity” is synonymous with “strict identity”, which (...) has nothing to do with simultaneity. “Diachronic Identity” is another designation for persistence of an ordinary thing through time and change. Now, strict self-identity holds independently of whether a thing has a short or a long duration. Hence, diachronic identity is synchronic identity. Lewis’ distinguishes two kinds of ontological analyses of persistence, the perdurance and the endurance analysis. This dichotomy is in several respects not exhaustive. Above all, his definition of “persist” is inadequate being based on the notion of multiple temporal localisation which is apt with interrupted but misplaced with persistent, i.e., temporally continuous objects. I sketch an explanatory framework that fits a variety of contemporary research programs in cognitive science. I then investigate the scope and the implications of this framework. The framework emphasizes (a) the explanatory role played by the semantic content of cognitive representations, and (b) the important mechanistic, non-intentional dimension of cognitive explanations. I show how both of these features are present simultaneously in certain varieties of cognitive explanation. I also consider the explanatory role played by grounded representational content, that is, (...) content evaluated by appeal to its truth, falsity, accuracy, inaccuracy and other relational properties. In this work, we focus on a very specific case study: assuming that quantum theories deal with “particles” of some kind, what kind of entity can such particles be? One possible answer, the one we shall examine here, is that they are not the usual kind of object found in daily life: individuals. Rather, we follow a suggestion by Erwin Schrödinger, according to which quantum mechanics poses a revolutionary kind of entity: non-individuals. While physics, as a scientific field, is (...) not concerned with whether entities posited by a specific physical theory are individuals or not, answering this question is part of the quest for a better understanding of physical reality. Convinced that "the role of philosophy in the advancement of science is to make trouble," Erwin Straus has led an informal group of college professors, permanent research staffs of the Lexington's psychiatric hospitals, and a parade of young government doctors, to challenge the foundations of their disciplines to come up with a synoptic view of psychiatry. In this book a French psychiatrist and an American philosopher join Straus in issuing the call to a wider audience. Straus finds that psychiatry (...) grew up within the mechanist tradition, with its split between mind and body. He urges the replacement of this model by a phenomenological one. This point of view is one link among the essays of the book. Another link is their prior publication in Psychiatrie der Gegenwart, Band 1/2, an international work intended to breach the walls between Continental and Anglo-Saxon psychiatries. Straus' paper is translated from the German by E. Eng, and Ey's from the French by S. C. Kennedy. Ey contributes a summary of his thirty-years work: to formulate an organodynamic conception of mental disease. Nathanson considers such questions as, What is the epistemic root for the concept of "normalcy" which psychiatry uses and builds upon?, and How is it possible that human reality has as one of its major expressions, the "abnormal"?--M. The 21 selections are divided into three conceptual approaches to the study of perception: the neurophysiology, the psychology, and the phenomenology of perception, with a final section, some problematic studies. In effect, however, the editor is challenging the metaphysical position hidden in the attitude that behavioral physiology should be an "exact science" without philosophical commitments. Parts II and IV, no less than the explicit statements of Husserl, Merleau-Ponty, Gurwitsch and Erwin Strauss in Part III, stand over against a point (...) of view which, beginning with Cartesian dualism, attempts to resolve it in a materialist reduction, a point of view in which behavior is always reaction, of nervous systems to physical stimulation from an "external," "real," world. Tibbetts is pressing two points--first, that all science must be based on some presuppositions or other and second, that any metapsychology ignores physiological and behavioral research only at its peril. The origin and the development of scientific disciplines has been a topic of reflection for several decades. The few extensive case studies support the thesis that scientific disciplines are not monolithic structures but can be characterized by distinct social, organizational and scientific–technical practices. Nonetheless, most disciplinary histories of genetics confine themselves largely to an uncontested account of the content of the discipline or occasionally institutional factors. Little attention is paid to the large number of researchers who, by their joint efforts, (...) ultimately shaped the discipline. We contribute to this aspect of disciplinary historiography by discussing the role of women researchers at the Institute for Heredity Research, founded in 1914 in Berlin under the directorship of Erwin Baur, and the sister of the John Innes Institute at Cambridge. This paper investigates how and why Baur built a highly successful research programme that relied on the efforts of his female staff, whose careers, notably Elisabeth Schiemann's, are also assessedin toto. These women undertook the necessary ‘technoscience’ and in some cases innovative work and helped increase the prestige of the institute and its director. Together they played a pivotal role in the establishment of genetics in Germany. Without them the discipline would have developed much more slowly and along a divergent path. (shrink)

This exploratory study assessed the use of functional Near Infrared Spectroscopy (fNIRS) to examine hemodynamic response patterns during sentence processing. Four groups of participants: monolingual English children, bilingual Chinese-English children, bilingual Chinese-English adults and monolingual English adults were given an agent selection syntactic processing task. Bilingual child participants were classified as simultaneous or sequential bilinguals to examine the impact of first language, age of second-language acquisition (AoL2A), and the length of second language experience on behavioral performance and cortical activation. Participants (...) were asked to select the agent of four types of sentences: subject-verb-object (SVO), passive (PAS), subject-extracted relative clause (SR), and object-extracted relative clause (OR) adopted from the “Whatdunit” task by Montgomery et al. (2016). Semantic cues were removed by using inanimate nouns for agents and patients, which constrained participants to make decisions based on syntactic knowledge. Behavioral results showed greater accuracy for canonical SVO and SR sentence types than for noncanonical OR and PAS sentence types, which aligns with prior studies. Neuroimaging results revealed greater hemodynamic responses to relative clauses (i.e., SR and OR sentences) than to simple sentences (SVO and PAS), especially for Chinese-English bilinguals suggesting first-language transfer influencing sentence processing in English. The effects AoL2A and the length of second language experience showed no significant differences between simultaneous and sequential bilinguals or between bilingual adults and children for identifying the correct agent in each sentence. However, neuroimaging results demonstrated greater hemodynamic responses in right dorsolateral prefrontal cortex (DLPFC) and left inferior parietal lobule (IPL) in simultaneous bilinguals compared to sequential bilinguals and greater hemodynamic responses in left and right DLPFC and left IPL among bilingual adults. Different behavioral and neural hemodynamic response patterns afford new insights into the effects of syntactic knowledge on sentence processing. (shrink)
